The articles reviews the problem of humanitarian terrorism that is a terrorism of self-proclaimed humanitarian goals and self-inflicted constraints. This type of terrorism justifies itself by lofty aspirations and claims that its actions are targeted killings of guilty individuals only. This terrorism is the product of the Enlightenment, it emerged by the end of the 18th century and passed three stages in its development. The first stage is the classical terror of the Jacobins 1793–1794. The second one is Russian revolutionary (...) terror of the end of the 19th – early 20th centuries. The third stage is the contemporary American warfare waged by the unmanned aerial vehicles, called drones. From the perspective of the contemporary just war theory, this terrorism is not only morally superior to the ordinary primitive terrorism of straightforward attacks on civilians (this terrorism may be no less fair in terms of self-imposed goals, but is doubtful in terms of means), but even contemporary war. Terrorists of this type kill the few but teach a lesson to many. But it must be clearly born in mind that humanitarian terrorism is not only the summit of just war but also the summit of absolute war. It is founded in personal and individual enmity, which makes the core of absolute enmity. Absolute enmity may at times be inevitable and even justified, but it blocks the road to peace. Revengeful spite, stemming from absolute enmity, is capable of creating its own phantoms of justice, propelling the war. The author concludes that the vicious circle is thus completed. The logic of just war drags in the direction humanitarian terrorism, humanitarian terrorism drags in the mire of absolute enmity. Absolute enmity proclaims just war. (shrink)

We continue the research of an extension of the divisibility relation to the Stone‐Čech compactification. First we prove that ultrafilters we call prime actually possess the algebraic property of primality. Several questions concerning the connection between divisibilities in and nonstandard extensions of are answered, providing a few more equivalent conditions for divisibility in. Results on uncountable chains in are proved and used in a construction of a well‐ordered chain of maximal cardinality. We continue the research of the relation $\hspace {1mm}\widetilde {\mid }\hspace {1mm}$ on the set $\beta \mathbb {N}$ of ultrafilters on $\mathbb {N}$, defined as an extension of the divisibility relation. It is a quasiorder, so we see it as an order on the set of $=_{\sim }$ -equivalence classes, where $\mathcal {F}=_{\sim }\mathcal {G}$ means that $\mathcal {F}$ and $\mathcal {G}$ are mutually $\hspace {1mm}\widetilde {\mid }$ -divisible. Here we introduce a new tool: a relation of congruence modulo an (...) ultrafilter. We first recall the congruence of ultrafilters modulo an integer and show that $=_{\sim }$ -equivalent ultrafilters do not necessarily have the same residue modulo $m\in \mathbb {N}$. Then we generalize this relation to congruence modulo an ultrafilter in a natural way. After that, using iterated nonstandard extensions, we introduce a stronger relation, which has nicer properties with respect to addition and multiplication of ultrafilters. Finally, we introduce a strengthening of $\hspace {1mm}\widetilde {\mid }\hspace {1mm}$ and show that it also behaves well with respect to the congruence relation. (shrink)

This text is a polemic against the 2023 article by Sergey N. Gradirovsky who wrote about the present-day relevance of Immanuel Kant’s concept of enlightenment and challenged the idea of the modern human being as a child who needs an external guardian or guide to control his behaviour. In my polemic with Gradirovksy I point out that in addition to “self-incurred immaturity” Kant writes about the historical “immaturity” of savage or backward peoples. I also argue that for Kant “maturity” carries (...) not only biological but also socio-historical connotations. I show that in the modern world Kant’s idea of the social or even historical maturity of the modern human acquires serious problematisation which was shown to be possible and inevitable in the article “What Is Enlightenment?” (1784) and its notion of the historicity of enlightenment, i.e. the very strategy of modernity is possible only when humankind reaches a certain historical age. Using as a point of departure the ideas of the French philosopher Michel Foucault, I maintain that the Kant­ian conception of enlightenment is at once ironic and tragic. It is ironic because it does not rule out that its main thesis on the historical maturity of the human being may turn out to be wrong. It is tragic because its main thrust is “heroisation of the present”, i.e. a readi­ness to resist the temptation of being absorbed by the future, which takes on added relevance in view of the virtualisation of reality. Traditionnellement, le musée d’art constituait un endroit dans lequel l’art du passé était collecté, conservé et montré au beau milieu de la vie contemporaine. Mais que signifie construire un musée d’art contemporain ? Whatever the case, the ready acceptance of the “museum of contemporary art” in today’s culture is significant. Background: Although convolutional neural networks (CNNs) achieve high diagnostic accuracy for detecting Alzheimer’s disease (AD) dementia based on magnetic resonance imaging (MRI) scans, they are not yet applied in clinical routine. One important reason for this is a lack of model comprehensibility. Recently developed visualization methods for deriving CNN relevance maps may help to fill this gap as they allow the visualization of key input image features that drive the decision of the model. We investigated whether models with higher accuracy (...) also rely more on discriminative brain regions predefined by prior knowledge. Methods: We trained a CNN for the detection of AD in N = 663 T1-weighted MRI scans of patients with dementia and amnestic mild cognitive impairment (MCI) and verified the accuracy of the models via cross-validation and in three independent samples including in total N = 1655 cases. We evaluated the association of relevance scores and hippocampus volume to validate the clinical utility of this approach. To improve model comprehensibility, we implemented an interactive visualization of 3D CNN relevance maps, thereby allowing intuitive model inspection. Results: Across the three independent datasets, group separation showed high accuracy for AD dementia versus controls (AUC ≥ 0.91) and moderate accuracy for amnestic MCI versus controls (AUC ≈ 0.74). Relevance maps indicated that hippocampal atrophy was considered the most informative factor for AD detection, with additional contributions from atrophy in other cortical and subcortical regions. Relevance scores within the hippocampus were highly correlated with hippocampal volumes (Pearson’s r ≈ −0.86, p < 0.001). Conclusion: The relevance maps highlighted atrophy in regions that we had hypothesized a priori. This strengthens the comprehensibility of the CNN models, which were trained in a purely data-driven manner based on the scans and diagnosis labels. The high hippocampus relevance scores as well as the high performance achieved in independent samples support the validity of the CNN models in the detection of AD-related MRI abnormalities. The presented data-driven and hypothesis-free CNN modeling approach might provide a useful tool to automatically derive discriminative features for complex diagnostic tasks where clear clinical criteria are still missing, for instance for the differential diagnosis between various types of dementia. The aim of this study was to assess nurses’ and physicians’ ethical dilemmas in clinical practice. Nurses and physicians of the Clinical Hospital Centre Rijeka were surveyed (N = 364). A questionnaire was used to identify recent ethical dilemma, primary ethical issue in the situation, satisfaction with the resolution, perceived usefulness of help, and usage of clinical ethics consultations in practice. Recent ethical dilemmas include professional conduct for nurses (8%), and near-the-end-of-life decisions for physicians (27%). The main ethical issue is (...) limiting life-sustaining therapy (nurses 15%, physicians 24%) and euthanasia and physician-assisted suicide (nurses 16%, physicians 9%). The types of help available are similar for nurses and physicians: obtaining complete information about the patient (37% vs. 50%) and clarifying ethical issues (31% vs. 39%). Nurses and physicians experience similar ethical dilemmas in clinical practice. The usage of clinical ethics consultations is low. It is recommended that the individual and team consultations should be introduced in Croatian clinical ethics consultations services. (shrink)

BackgroundThe UN Convention on the Rights of Persons with Disabilities, and the reformed guardianship law in Germany, require that persons with a disability, including people with dementia in Alzheimer’s disease (PwAD), are supported in making self-determined decisions. This support is achieved through communication. While content-related communication is a deficit of PwAD, relational aspects of communication are a resource. Research in supported decision-making (SDM) has investigated the effectiveness of different content-related support strategies for PwAD but has only succeeded in improving understanding, (...) which, although one criterion of capacity to consent, is not sufficient to ensure overall capacity to consent. The aim of the ‘spatial intervention study’ of the DECIDE project is to examine an innovative resource-oriented SDM approach that focuses on relational aspects. We hypothesise that talking to PwAD in their familiar home setting (as opposed to a clinical setting) will reduce the complexity of the decision-making process and enhance overall capacity to consent. MethodsPeople with a suspected or confirmed diagnosis of dementia in Alzheimer’s disease will be recruited from two memory clinics (N = 80). We will use a randomised crossover design to investigate the intervention effect of the decision-making place on capacity to consent. Besides reasoning capacity, which is part of overall capacity to consent and will be the primary outcome, various secondary outcomes (e.g., other aspects of capacity to consent, subjective task complexity, decisional conflict) and suspected moderating or mediating variables (e.g., meaning of home, demographic characteristics) will be assessed.DiscussionThe results of the study will be used to develop a new SDM strategy that is based on relational resources for PwAD. If a change in location achieves the anticipated improvement in capacity to consent, future research should focus on implementing this SDM strategy in a cost-effective manner in clinical practice.Trial registration: DRKS00030799.
